Directions for completing your Annual Faculty Activity Review

1. This is a WORD (2003) document. Save it to your H:drive, using a file name that includes your initial, last name, review, and year (e.g.., jsmith-review-12-13.doc).
2. The timeframe for the current year is the period since the prior evaluation, i.e., approximately March 2012 to March 2013.
3. Enter "N/A" for any question that does not apply to you. This indicates to the reader that you did not miss the question
4. When the document is complete, save and close. Attach the document to e-mail and send it directly to your Department Chair or Campus Assistant Dean.
