Policy

All faculty are required to provide written proof of the following at the time of employment and annually or upon expiration thereafter:

  1. Written verification of current certification in Cardiopulmonary Resuscitation (CPR) (Basic Life Support for Healthcare Professionals).
  2. Written verification and documentation of current immunizations listed below:
    1. PPD for tuberculosis; annually unless individual tests positive. If test is positive, current CDC guidelines will be followed (www.cdc.gov).
    2. INFLUENZA immunization annually.
    3. HEPATITIS B series of immunizations and/or adequate titer levels.
    4. TETANUS Tdap vaccination 1x an adult; booster (Td) every 10 years thereafter.
