<p>The following steps are required for the proposed change in concept or exemplar.</p>
<li style="margin-bottom:15px;">This is an editable form. Save the form using the following convention: F.Nightingale_2016. Use the year in which the evaluation form has been completed.</li>
 
<li style="margin-bottom:15px;">The timeframe for the current year is the period approximately January 1 to December 31 of the previous year.</li>

<li style="margin-bottom:15px;">CV should be in the currently approved UNMC format. Updates, since the previous evaluation year, should be highlighted in yellow before submitting your self-evaluation.</li>

Faculty may request a change in either a concept or exemplar in a current course in written form to the appropriate curriculum committee. If approved by the committee, the change will be made in the course at the appropriate time (e.g., next offering). Faculty must follow the guidelines and use the form provided below.
