===Directions for completing your Annual Faculty Activity Review===
<ol style="margin-bottom:15px;max-width:70em !important;">
<li style="margin-bottom:15px;">This is an editable form.  Save the form using the following convention: F.Nightingale_2016. Use the year in which the evaluation form has been completed.</li>
<li style="margin-bottom:15px;">The timeframe for the current year is the period approximately January 1 to December 31 of the previous year.</li>
<li style="margin-bottom:15px;">CV should be in the currently approved UNMC format. Updates, since the previous evaluation year, should be highlighted in yellow before submitting your self-evaluation.</li>
<li style="margin-bottom:15px;">Send completed form, your updated and highlighted CV and other supporting documents to your designated supervisor.</li>
