Directions for completing your Annual Faculty Activity Review

  1. This is a WORD (2003) document. Save it to your H:drive, using a file name that includes your initial, last name, review, and year (e.g.., jsmith-review-12-13.doc).
  2. The timeframe for the current year is the period since the prior evaluation, i.e., approximately March to February of the following year.
  3. Enter "N/A" for any question that does not apply to you. This indicates to the reader that you did not miss the question
  4. When the document is complete, save and close. Attach the document to e-mail and send it directly to your Division Assistant Dean.
