Directions for completing your Annual Faculty Activity Review

1. This is an editable form. Save the form using the following convention: F.Nightingale_2016. Use the year in which the evaluation form has been completed.
2. The timeframe for the current year is the period approximately January 1 to December 31 of the previous year.
3. CV should be in the currently approved UNMC format. Updates, since the previous evaluation year, should be highlighted in yellow before submitting your self-evaluation.
4. Send completed form, your updated and highlighted CV and other supporting documents to your designated supervisor.
