Directions for completing your Annual Faculty Activity Review
- This is an editable form. Save the form using the following convention: F.Nightingale_2016. Use the year in which the evaluation form has been completed.
- The timeframe for the current year is the period approximately January 1 to December 31 of the previous year.
- CV should be in the currently approved UNMC format. Updates, since the previous evaluation year, should be highlighted in yellow before submitting your self-evaluation.
- Send completed form, your updated and highlighted CV and other supporting documents to your designated supervisor.
