<li style="margin-bottom:15px;">This is a WORD (2003) document.  Save it to your H:drive, using a file name that includes your initial, last name, review, and year (e.g.., jsmith-review-12-13.doc).</li>
<li style="margin-bottom:15px;">This is an editable form.  Save the form using the following convention: F.Nightingale_2016. Use the year in which the evaluation form has been completed.</li>
<li style="margin-bottom:15px;">The timeframe for the current year is the period since the prior evaluation, i.e., approximately March 2012 to March 2013.</li>
<li style="margin-bottom:15px;">The timeframe for the current year is the period approximately January 1 to December 31 of the previous year.</li>
<li style="margin-bottom:15px;">Enter "N/A" for any question that does not apply to you. This indicates to the reader that you did not miss the question</li>
